Vaccine Booster Clinic

The University is offering a vaccine booster clinic, in partnership with local pharmacy SunRay. Those who wish to receive a COVID-19 booster can sign up via the COVID-19 Vaccine Clinic page. A booster shot is required for students, faculty, and staff by Jan. 31.

8:00 a.m. - 4:00 p.m.

Gimble Gym, 3701 Walnut St.

A COVID-19 Vaccination Record Card, which reads, "Please keep this record card, which includes medical information about the vaccines you have received. Por favor, guarde esta tarjeta de registro, que incluye informacion medica sobre las vacunas que ha recibo. Last Name, First Name, MI, Date of birth, Patient number (medical record IIS record number). Vaccine, Product Name/Manufacturer, Lot Number, Date, Healthcare Professional or Clinic Site,” as well as 1st dose, 2nd dose, and other.
